We are delighted you wish to apply for the 5-day training course. Please complete the following steps:

2. Complete the form fully, making additional documents as needed
3. Email completed form and additional documents to QPFT@theqeffect.com on or before November 1, 2010 (or until seating is full)
4. Once your application has been approved, register for the workshop online (your seat will be reserved when your payment has been received.)
